A form is a small public page that collects a visitor’s name, email or mobile number. It belongs to a location or to a brand, takes its colours, font and logo from the brand appearance, and every submission lands in Contacts as a new or updated record. Use it wherever you want to capture a lead without a conversation: a QR code on the counter, a link in a social bio, a newsletter sign-up.

Before you start

  • Forms use the Microsites permission module — there is no separate forms permission. Restricted access lets you open forms read-only. To create, edit or delete a form you need full access on the location or brand that owns it. See Users and permissions.
  • Colours, fonts and the logo come from the brand appearance in Settings → Brands — see Brands.
  • Either the email or the mobile number must stay required, so that every submission can be matched to a contact. The editor locks the last required one.

Creating a form

  1. Go to Web pages → Forms.
  2. Click Create form.
  3. Fill in the Internal name. It is only shown in the dashboard and in contact timelines.
  4. Choose the Location or Brand that owns the form.
  5. Write the Title, an optional Description and the Confirmation message the visitor sees after submitting.
  6. Under the contact fields, switch on Name, Email and Mobile number as needed and tick Required where you want it.
  7. Optionally pick the Segments every submitter should join — see Segments.
  8. Check the preview on the right and click Save.

Use Localization to translate the title, description and confirmation message. A visitor sees the version matching their browser language, or the account language when there is no translation.

Sharing the form

Open a form and copy its Public link from the editor. The copied link always opens the form on a Pluspoint URL — a form has no custom domain of its own. A Form button on a microsite is different: it keeps the visitor on whatever domain that microsite is served from, including a custom one. A form owned by a location attaches that location to everyone who submits it. A form owned by a brand does not attach any location — unless it is opened from a Form button on a location microsite, which carries that location through to the submission.

What a submission does

  • Pluspoint looks for an existing contact in your account with the same email or mobile number. If there is none, it creates one.
  • The segments chosen on the form are added to the contact.
  • If a location owns the form, that location is added to the contact.
  • The contact’s timeline gains a Form submitted entry naming the form, and a Contact created entry when the contact is new.
  • An automation whose trigger includes Form submitted can enrol the contact — see Automations.
  • The visitor sees the confirmation message. Their name, email and mobile number are remembered in the browser, so a later survey or live chat on the same device is prefilled.

Where submissions show

Each card shows Views and Submissions. The download icon on the card exports every submission as a CSV — one row per submission, with the name, email, phone number, location, time and the segments that contact now carries. Dates are in UTC.

There is no submissions list in the app. To see who submitted without exporting, open Contacts and filter by a segment the form applies, or look for Form submitted in a contact’s Activity tab — see Managing contacts.
