QR code surveys
Updated August 24, 2026
Every survey template has a permanent link and a matching QR code. A customer scans a sticker on the table, a receipt or a door, then answers on their phone without typing anything. QR codes are the cheapest way to collect feedback at scale: there is no channel to configure, no contact list to build, and no per-message cost.
Before you start
- To change the design you need Surveys access at the full tier, on a team whose scope covers the template’s location or brand. Read-only access still lets you open the template and copy the URL.
- A survey template must exist. See Creating survey templates.
- Connect the profiles for the review sites you want the survey to offer, or the review screen will have nothing to show.
- Building and downloading QR codes works on plans without full surveys too. On those accounts the sidebar item is called QR Codes. Collecting ratings, adding custom questions and sending invitations are locked behind an upgrade prompt — the QR is the whole feature.
Where to find it
Surveys → Templates (https://app.pluspoint.io/surveys/templates) → the edit icon on a template card → the Sharing tab.
On an account where the section is called QR Codes, the same grid sits at https://app.pluspoint.io/surveys and the sub-pages are not available.
What a customer sees after scanning
The QR opens the public survey. What happens next depends on the template’s Collect Customer Rating setting.
| Collect Customer Rating | Flow |
|---|---|
| Off | The customer lands directly on Leave a review with your review-site buttons. Nothing is asked, nothing is stored beyond the visit. |
| On | The customer rates their experience and answers any custom questions. A customer whose rating is below the template’s threshold is also asked How can we make things better?, where the written feedback stays private |
Collect Customer Rating is off on a new template, which makes an unmodified QR a pure review-generating code. Turn it on in the Settings tab if you want ratings, questions and written feedback when a rating is low.
If exactly one review site is available, the review screen redirects to it automatically after a moment rather than showing a single button.
Copy the survey link
- Open the template and go to the Sharing tab.
- Under Survey URL, use the copy button on the read-only URL field.
| Template type | Link shape |
|---|---|
| Location template | https://plsp.io/q/<externalId> |
| Brand template | https://plsp.io/q/<externalId>:<locationId> |
The identifier never changes for the life of the template, so a printed code stays valid however often you edit the wording.
Note On a brand template, use the Location dropdown above the URL field to choose which location the link and QR point at. Generate one code per location. A brand link without the location suffix still works, but the customer is stopped by a Which location did you visit? prompt before they can answer.
Customise the QR design
With Customize QR design off you get the standard Pluspoint code: the Pluspoint logo in the centre, a dark teal fill on a white background, and no text. Turn the toggle on to brand it.
| Control | What it does | Limits |
|---|---|---|
| Logo | Replaces the centre logo | Pick any image file and crop it square; it is saved as a 250 px PNG. That cropped PNG has to come out between 10 KB and 500 KB |
| Top text | A line above the code | Up to 42 characters, wrapped onto at most two lines |
| Bottom text | A line below the code | Up to 42 characters, wrapped onto at most two lines |
| Background | The poster and code background colour | Required, HEX, for example #ffffff |
| Fill | The colour of the code itself and of the top and bottom text | Required, HEX, for example #1f4c5c |
Leave both Top text and Bottom text empty and you get a compact code with no poster frame — useful for a sticker or a corner of a receipt. Fill either one in and the code is rendered as a poster with your text around it.
Keep contrast high between Fill and Background. Codes are generated at the highest error-correction level, so they survive a centre logo and a bit of print damage. A low-contrast pair still fails to scan.
Download or print
| Action | Result |
|---|---|
| Download → PNG | Saves QR.png — use it for anything on screen, or for simple print jobs |
| Download → SVG | Saves QR.svg — vector, so use this for large-format print, signage or a designer |
| Download → PDF | Saves QR.pdf — print-ready as-is |
Save the template before you print a run. The preview and the downloaded file redraw from what is on screen, but the survey the code opens is the one you last saved. A rating setting or a question you have not saved yet will not be there when someone scans.
Where to put the code
- On the table, the counter, the receipt or the invoice — anywhere the customer is already sitting still.
- On packaging, delivery bags and product stickers.
- On the door or window as they leave.
- One code per location on a brand template, so responses land against the right location without asking the customer.
Measure how a code performs
Every scan is recorded as a visit. Unlike a sent invitation, a QR records a fresh visit each time it is opened. Two places show the results:
- The template card on Templates shows Views: and, when the template collects ratings, Responses:.
- Surveys → Analytics → Conversion has a dedicated QR codes table, broken down by location and expandable per template.
Proceeded to review sites is the honest measure of a QR’s effect, because review sites do not disclose where a review came from. See How QR code conversion is measured.
Over-survey protection never applies to QR scans, so a regular customer can scan and answer as often as they like.
Troubleshooting
| Symptom | Cause | Fix |
|---|---|---|
| The code goes straight to the review sites without asking anything | Collect Customer Rating is off | Turn it on in the template’s Settings tab |
| Scanning shows “Which location did you visit?” | The link is a brand link with no location in it | Pick the location in the Location dropdown on Sharing, then copy or download again |
| The review screen shows no buttons | None of the ticked platforms resolves a review link for that location | Connect the profiles. Instagram needs an Instagram URL on the location’s social links |
| The logo upload is rejected | The cropped PNG fell outside 10 KB to 500 KB | Crop a different area or start from a different source image — the size check runs on the cropped result, not on the file you picked |
| The preview has stopped refreshing | It only redraws while the form is valid | Fix the validation error highlighted in the form |
| ”Couldn’t download” or “Couldn’t print” | The code could not be generated | Check the form for errors, save, then try again |
| A printed code will not scan reliably | Too little contrast between Fill and Background, or the code was printed too small from the PNG | Increase contrast and reprint from the SVG or PDF |
| Scanning shows a service-paused screen | The account’s subscription has expired | See Billing and plans |
| Scans are counted but no responses arrive | The template does not collect ratings, so there is nothing to submit — visits are still recorded | Turn on Collect Customer Rating if you need answers, not just review traffic |
